After completing your application on-line or PACK I below, we advise you to not fill out any remaining documentation until you are contacted by an Emergi-Staff Nurses recruiter. If your application meets our qualifications, we will instruct you to complete PACK II and III below.

In order to process your application, please make sure all forms are completed in their entirety. Once completed, please fax, mail or e-mail as attachments to the Emergi-Staff Nurses office. Testing materials and skill evaluations will be forwarded to you by your recruiter.

1. Performance Evaluation -We require a recent (within two years) clinical performance evaluation. If your past or current employer does not release performance evals, please notify the office.
